drivers are version 5.1.2600.XX Current solution is to
remove SP1. Any suggestions / solutions? Thanks!

Thank you for these suggestions, but they are ineffective.
After installing SP1A, I have no USB support (no mouse or
cable modem) either with or without the hotfix from KB
822603.
The only solution has been to remove SP1A.
Do you have any other suggestions?

Open your BIOS and check the settings for USB devices.
You might want to try setting your BIOS to all the default
settings.

All USB BIOS settings are set to default.

NOTE: WinVer reports the following:
Windows XP Professional
Build 2600.xpclnt_qfe.021108-2107

Appears to be a client QFE build?
System is from Fry's, P4 2.8 GHz with HyperThreading.
Does this help? Please advise.
Thanks,
Greg
