Look of form on return

Is it not possible to have a form return by email looking
like the form? I have tried everything and do not seem to
be able to get it looking proper. Also on the return
email the fields are all mixed up is there a way to change
this so they stay the same as the tab order?
 
Part A: No
Part B: If you have checked "Include field names on the email options tab,
they should be returned in the order they are on the form.

Correct, it is not possible to do what you want when using the FP Form Handler.

You will need to use a custom server-side form handler written in ASP, PHP etc. depending on what is
supported on the server your site is hosted on.
