Logon, Logoff, then Logon again in Safe Mode *!

It's been a couple months since I used Safe Mode, but when I did
yesterday (XP Pro SP2), this is what happened:

Boot with F8 for Safe Mode
At prompt window for Logon/password
I entered Administrator with blank for password
After 1 second I saw it start logging on, and
then Logoff. Then

Prompt window for Logon/password again
I entered Administrator with blank for password
PC then proceeded into safe mode.

Any idea why I am getting 2 logons? I assume that something failed
and forced a logoff. I turned on auditing and saw the two logons in
the Event Viewer, but still can't figure out why I get the
logon-prompt TWICE.

Any idea or suggestion on how to determine what is happening?

Is this happening when you aren't in safe mode?
I'd do malware scans.

No, it only happens in safe mode. I have done several malware scans
and it looks clean.

The Security event viewer shows logon event, 528 event, logoff, and
then logon again, confirming what I see on the screen.

Are there any trace logging tools that work in Safe Mode?
