XP includes a Dvorak keyboard layout for right hand typists. Directions for
setting this up can be found here:
http://www.microsoft.com/enable/training/windowsxp/keyboardlayout.aspx

Downloadable lists of keyboard shortcuts for every Microsoft product:
http://www.microsoft.com/enable/products/keyboardassist.aspx

A download from Microsoft for creating your own keyboard layout:

Pictorial of the available layouts in Windows (also an FAQ and some
additional software utilities in the right hand sidebar):

Haven't found anything on the above sites for combinations such as
Ctrl+Alt+Del yet. Perhaps the tool for creating a keyboard layout would
help there?

Also there are 'macro' programs that allow you to assign a series of
keystrokes to a "word" or typed sequence. For example: Using one of these
programs, typing #CAD would perform the same operation as pressing the
Ctrl+Alt+Del combination. If you do an internet search for these include
"recorder" and "macro" in your search terms.

I find onscreen keyboards with the traditional layout to be less than
"okay" for everyday use. I've used Message Ease with my Tablet PC and find
it very convenient and easy to use: http://www.exideas.com/

There's probably some hardware solutions: keyboards built with alternative
input patterns such as Frogpad (http://www.frogpad.com/).
