item_customaction (sue's vacation request)

I am trying to use sue moshers vacation request example, and am having trouble with the reply. I can create a request and send it however the reply does not seem to be executing the item_customaction event. The code to dislpay the calendar is working in the request, so I don't think it is a one-off form problem, however, I'm not 100% sure. Has anyone else seen this before

Where did you publish the form? Did you leave the "send form definition with item" box unchecked?

I installed it into the personal forms library, and left the "send form.." box unchecked. I sent the vacation request to myself, and then approved it. The approval never attached the calendar item to the message. I also modified the code in the intem_customaction routine to see if the code was ever executing, and it was never executed. I am using OL2000

Then something else may be causing the item to one-off. Check the size to
see if it's unusually large and the message class.
