Has the installation of Windows Vista Home Premium version gone smoothly
without damaging existing files on the hard drive?

Are there any reports of files lost during the installation process?

Yes except for when it hasn't. I know that doesn't sound terribly helpful
but then with all due respect the question is a bit of a silly one.

When the upgrade process works then it works well enough; but there will
*always* be problems, things that were not anticipated by the people who
designed the upgrade process, etc.

I personally have not had good experience of the upgrade process for Vista,
but I am probably not a typical home user, so this is no guarantee that you
will (or won't) have problems.

See above answer. But let me make this perfectly clear.

Anyone who does not backup before performing an upgrade is a complete fool
who deserves all they get if they lose their files.

I'm sure you'll ignore this answer because it probably isn't what you wanted
to hear, but I'm sure lots of other experienced people reading this will be
able to tell you I'm just telling the truth.

In theory you should not loose data...
but I've been reading reading a Vista news group and noticed some users have
lost data.
Though it's more than likely an error on their part...
You should *never* under any circumstance attempt any upgrade without
at least a full (and tested) backup of your data.

My real advice is to stay with XP (assuming it's working ok)
and not even think of upgrading to Vista
