Inserting files/text etc into form

I made a custom form, but I would like to be able to add file (like the insert>file menu) but I can do it with a text box. What Control should I use on my page to do this?

You need to use the large item body control, listed as Notes or Message on
the FIeld Chooser, depending on the type of form.
