Insert FIELD or ASK field - Help required!

Hi All,

I am not sure someone hasn't already answered this but.... I can't seem to
get the ASK or insert fields to work in my document. What I would like to do
is have a prompt box open that the user must fill in before the document
actually opens. Any help would be greatly appreciated!!!!!
