Insert Check Box

I'm revising a form in MS Word to only allow certin items to be checked off.
How do I insert a check box/radial button or something similar into a word
document?
 
Hi Shae,

CheckBox formfields are available on the Forms toolbar (which you may have to activate, via View|Toolbars). For this process to
work, your document will also need to be protected (via Tools|Protect Document| > Forms).
