Index in Word for proper names

I want to make in index in Word for names where the text is First Name
Surname and the resulting index is Surname, First Name. Any ideas as to how?
I am using Word 2002.
 
You have to construct the XE field as { XE "Surname, First Name" } If you
want all the surname entries grouped, use { XE "Surname:First Name" }.
