Identifying the entity ...

Hi all. Can someone help me identify the right entities with their attributes
from scenario below?

HelpFit LTD runs a chain of fitness clubs in Kano. The Fitness club provides
exclusive fitness training for its customers. The club has so far maintained
its records manually but is now looking to setup a database application to
support its functioning.
Each fitness centre is managed by a Centre manager. Each centre employs a
number of Instructors.
Customers need to first register at a centre by completing an application
form which requires them to provide their personal details including name,
Sex, IC/Passport No, date of birth and existing health issues if any.
Upon registration, a personal fitness Instructor is assigned to the customer
who provides training for the duration of the training course.
Lessons are of two hour duration and are conducted from 8:00 AM to 10:00 PM
everyday. Customers get a discount of 25% if they enroll for the fitness
lessons for off-peak hours (Weekdays 9:00 AM to 6:00 PM)
After each lesson, the instructor updates the number of hours of fitness
training taken by the customer.
HelpFit also has an incentive scheme for customers who have taken more than
50 hours of training in a year. They are entitled to one discounted spa
voucher.


I want to an Access2000 (or above) database with my own data model. However,
feel free to add attributes or tables if it is necessary.

Not actually, I tried several time but fail to identify them. Honestly is an
assignment but has different parts. this is only the beginning.

since this is a class assignment, surely you've had some classroom
instruction
and/or reading material to instruct you or give you the resources to figure
it out. for further resources, see the links at

i doubt that anyone here will simply tell you how to do the assignment; in
a written forum, that's tantamount to doing it for you. suggest you use the
resources at the above website, work on it yourself, and post the
entities/attributes you've come up with, if you get stuck. probably
somebody will be able to give you a nudge in the right direction so you can
complete the assignment on your own and claim the grade within the school's
honor system parameters.
