Icon in Phone List View

I've modified a new Contact form by adding some new fields to it. However,
when I now add a new contact, the icon that appears against the new contact
record in the Phone List View is that of a Note icon and not a Contact icon.

How can I change the default icon setting for new contacts I record, and
also for the ones I've since added that have the wrong icon?

Sounds like you didn't add an icon to your form on the (Properties) page in
design view.
