I need to create a template

Here is my situation. I am military, and operate a help desk for technical
support. My situation is that I want to create a Microsoft Word
template/work order form(basically something to describe the problem, user,
etc.) and some of my fields MUST contain three rows of underlines, which is
just for when we print out said document, we are able to pencil or ink by
hand information about what we did to fix the problem that day. My problem
is when trying to input data into the underlined sections, the underlines, or
rather underscores that I used to simulate the underlines, start to shift
right, or left when I undo the automatic insert. I will include an example
of what I am talking about so that whoever can help me correct this problem
gets a better understanding of what I am talking about:
(Before text is imputed

(after text is inputed)
Problem:_(Random data)
