In Word, select Tools | Letters and Mailings | Envelopes and Labels. On the
Labels tab, click Options... With Avery Standard selected, scroll down to
5388 - Index Card and select it, OK. Click New Document to create a full
sheet. They're set up as a table, so make sure you have table gridlines
displayed (Table | Show Gridlines) so you can see the card boundaries.
