Hyperlink colour inconsistent - some don't change

I have a macro to run on documents, to find and convert text enclosed by square brackets to hyperlink to a Notes document. It has been running fine, but now we have a document where the hyperlink colours are not consistent. The text still gets hyperlinked, some changed to the blue+ underline, some remain as is (ie. black + bold).

The style before the conversion is Palatino, after conversion the style shows as Hyperlink. Reapplying the style does not change the colour either. I cannot see what is different between the ones that were changed and the ones that didn't

Any clues?

If you've visited the target page recently, the character style applied is
Followed Hyperlink rather than Hyperlink; but I've seen exactly this problem
also. Seemed to me it was (at least sometimes) dependent on the actual
hyperlink colour: the problem disappeared if hyperlink was defined as green
rather than blue.
