HTML Form

I am looking to send an HTML form in an email to extract information from clients. (I send the form, they hit Reply, fill in the form fields and then send it back.) I want to parse this information into a MS Access database. Can anyone give me a hint as to what to use or how to go about this?

An Outlook form can be used for this internally in an Exchange Server organization, but it is not a viable solution for external clients. Consider using a web page instead.
