HP 3015 Laser ghost printing and toner smearing

As the subject line reads, if I print something as simple as a text
document, the text is "ghost printed" a second [and third time].

Additionally, extraneous toner is smeared on the page.

Here is an example - only one line of text, "Start Text End" exists in
the single-page document (I've scanned the printed page in, then
"boxed" the real text in red) ... notice that the text, "Start Text
End" is repeated a second time (ghost print) and take note of the
smeared toner (the toner is fused to the paper).

Any thoughts what the problem is?

HP LaserJet 3015, about 2 years old, orig toner cartridge.

Common symptoms of a failed toner cartridge. Some HP toner
cartridges have a lifetime (untill all the toner is used) warranty.
Getting the warranty honored a major hassle, but can be successful
if you tackle it the right way.
Cartridges have a scavenger blade to remove excess toner from
the drum, so it doesn't go round again, and produce
ghost images. It also should remove the toner that's producing the
smearing.
