How to set up electronic book icon for mailing envelopes in WORD

The electronic book icon does not appear in either the delivery address or
the return address boxes.
 
What version of Word are you using?

If Word 2007 the Address book link requires Outlook 2007 to be installed, if I recall correctly.

I'm using Outlook Express for email, and in Word 2007 I get a normal mail
header with the Address Book icon in the expected places. But if the OP is
using Outlook as a mail client and it is not Outlook 2007, that might make a
difference. I have Outlook 2003 rather than 2007 installed, but since I'm
not using it for mail, I can't test this.
