How to remove programs from "Open With" dialogue?

When I attempt to open a file that does not have an extension, the "Open
With" dialog box presents me with a list of programs from which to
choose to open this extenionless file, or I may choose to click the
"Browse" button.

Unwanted programs appear on this list. One such program is:

"PerVis NT Performance Tool"

also known as "Bootvis.exe"

I tried using Regedit to edit the branch:
HKEY_CURRENT_USER\Software\Microsoft\Windows\CurrentVersion\Explorer\FileExts

However, this program was not to be found in any of the OpenWithList
folders of any of the file types listed there. In fact, no reference to
this file could be found in any part of this branch.

The only reference to "PerVis NT Performance Tool" in the registry was
to be found in various Muicache folders, four, to be exact. After
backing up those folders, I deleted every reference to "PerVis NT
Performance Tool" from the registry and still the program appears in the
"Open With" dialog box. I restored the deleted references.

At this point, I would like to stress that this is *not* the right-click
context menu I am concerned with. I can edit that with no problem.

It is the "Open With" dialog box that appears when clicking on a file
with no extension or no known extension.

Make that "PerfVis NT Performance Tool"
