Assuming your printer can handle the non-standard size paper, in Page
Setup (2003: File > Page Setup; 2007: double-click on the ruler) on
the "Paper" tab choose "Custom" and enter the dimensions.
In your Print dialog (Ctrl-P), click Printer Properties, and somewhere
you'll find the list of possible paper sizes (differs with every model
of printer).
Many printer drivers are poor with custom paper sizes and will adopt the
nearest standard size to your selection. The simplest solution is to use the
next larger standard size (or use the default page size) and make up the
difference with the margins.

If these are tent fold cards, use a two cell table and rotate the text in
each cell one to the left, the other to the right.
