There are several software programs that can create fillable PDF forms that are lower priced than Adobe's Acrobat. While not quite
free, there is CenoPDF
http://lystech.com ($39.95) and you can download the eval version (which adds a watermark on each page until
paid) to see if it will do what you need.
Another ($19.99) is PDFill (
http://PDFill.com) from PlotSoft.
One of the better known packages is FoxIt Editor from
http://foxitsoftware.com/pdf/pe_intro.php a bit pricier.
