How to look up records from Table????

I have a table with following fields: Assembly P/n#, Unit Price, and
Assembler. I want to create a query form that allow me to search for the Unit
Price and Assembler when I enter the Assembly P/n#. Can someone help me with
this issue??? I really need this for my small business.
