How to limit one answer per question on a form?

I created a survey in MS Word with check boxes from the Form toolbar. The
questions ask to rate performance on a scale of 1 to 5. How can I restrict
the end user to only answering one item per question before I lock the form?
Example:
Question 1 pertains to Availability:
_ 1 = Difficult to reach, on site and remotely.
_ 3 = Usually available on site or remotely.
_ 5 = Always there when needed; flexible and accommodating.

Alternatively, put the choices in a dropdown field.
