How to insert MSReader into word menus/toolbar?

This was done automatically in Word 2000 but I can't seem to get it in Word
2003. Any suggestions?
 
MS Reader - what's that?

Word 2003 uses a different startup folder location from Word 2000. Any
add-ins that remain in the original location will not be loaded. Note too
that some 2000 add-ins will require upgrading to work with 2003.
