How to get rid of a document "stuck" on the desktop.

Every time I log in, a document (that has never been saved on the "c" drive)
pops up on the screen. It's "stuck" there. If I click the "new" icon,
another copy of the document appears. In order to use a new document, I have
to click and drag to highlight the text of the document and then delete.
This is a nuisance and after trying numerous things to solve the problem
(including trying to save the "stuck" document), nothing seems to work. Even
a tech service person has not been able to help.

.. . . and, what combination of keys did I accidentally hit that caused the
problem in the first place?
