How to create secure forms by Word 2003?

I would like to create a secure form by using Microsoft Word 2003. People who
will be using my form can only input their data in my document. They can't
change the question or the texts on the document.

I know this is possible with Microsoft Word 2003 because I have found many
premade templates. I really want to know how to create such documents myself.
