How to create MeetingItem in Outlook?

I'm processing a calendar using the Outlook9 api and encountering a
MeetingItem. In order to code the handling of this item properly, I've
been trying to create some test data. However, I can't get Outlook 2003
to create a MeetingItem ... everything shows up as AppointmentItem,
even if the MeetingStatus field is set to olMeeting. (I'm using
OutlookSpy to determine this.)

Have MeetingItems been deprecated, or is there some way to create them
using OL2003?
