I have a document that was created by someone else and now a table of
contents and index needs to be added. The direction provided seem simple but
I am unable to insert the table to contents or the index.

Please assist - Thanks!

See http://addbalance.com/usersguide/complex_documents.htm and
http://www.shaunakelly.com/word/numbering/UseBuiltInHeadingStyles.html. Word
will quickly generate a table of contents for you. The easiest way to have
this happen is to use the built-in heading styles for the headings you want
to include. (You can change how these styles look.) For an index, you need
to mark the terms you want in your index. The first article above shows how
to do both.
