How To: Create a password reset page

I would like to create a webpage to allow my users to reset their passwords
after going through a verify checklist. My user credentials are stored in a
sql db. How do I create a form that can check fields like birthdate or
address and allow them to reset their password.
 
You will have to learn to use whatever scripting language is currently being used on the site to
write code to do what you want.
