Need to know if any one has any format of an After Action Report?

Yes, we all do 
But you need to ask a question here before we can give you an answer. If
your question was going to be "Would we please send you a copy of our report
format?" the answer from most of us is likely to be "No".
If you ask us how you could MAKE such a report, we'll be a lot more help.
Generally, we're happy to help around here, but we won't do it FOR you

I disagree. I have no idea what an after-action report is. If it were
explained to me, I might be able to offer suggestions, but I agree that the
