How to connect seperet plug bits of case USB to motherboard?

Hi,

Bought a new case which has 2 front USB's. However, the wires from the
USB's each have 4 separate connectors not like a speaker connector
that is a row of 4 connected together.

The markings on each USB cable ending are as follows:
-D
+D
VCC
GROUND

My motherboard asus a7v8x has a socket for 2 extra usb ports to which
I want to connect the case usb's.
The markings in the mboard manual are:

GND
LDP6
LDM6
USB+5V

Which case connector to which mboard socket?
