How do you add Drop-Down to Documents that...?

I am trying to created a company Fax Form. In the From: and Subject: I want
to restrict the choices to preselected list of information. As for the
everything else on the Fax Document, People can enter any information freely.

An alternative to Doug's suggestion is using AutoTextList fields:
