How do I use a button to open and filter two fields on another.

I have one form with multiple fields...I created a button to pick the data
from one field and open another form filtering based on the data in that
field (simple). What I'm trying to figure out is how to open a form based on
filtering two fields when I click that button.

Please post back with how you are opening the form with one field (code
please) and I think I can show you how this works. It isn't that difficult.
