How do I set up forms completed in word on a stand alone pc?

I have created a form with a customized schema attached. I want to complete
it in word on a stand alone pc. I want to have the data saved and the form
cleared to receive the next persons data?
 
If you create the form as a Template, then every time a new Document is
created based on it, the form is empty (or otherwise as you design).
 
What you are talking about is what Word calls an "online form." Check this

Best method I can think of is to save your form as a template and create a
new document based on it for each use. This gives you a clean form each
time. Set it to save data only.
