How do I set up autocorrect of more than one line of text in word.

I am doing medical transcription and for the life of me I cannot figure out
how to save my frequently used sentences and/or multi-line text for automatic
playback based on keying in a few charaters....like autocorrect but text that
allows returns
for multiple lines. For example, if I key in SK space, I would want it to
play back

Dr. Shilo Kabir
1210 Blue St.
Anywhere, TX 60006

Dear Dr. Kabir:


I have done this many times in the past but cannot remember where I go to
set it up. Any help appreciated. I feel like I am missing something really
simple in this software application.

Select the text, then from the Tools menu, select AutoCorrect Options and
check the Formatted text radio button and enter the text that you wish to be
replaced by the selected text.
