How do I scan a paper document into a Word file?

You need optical character recognition (OCR) software. Most scanners
come bundled with some kind of OCR program... Textbridge and Omnipage
are two popular examples.
 
There is even a rudimentary OCR application included with Office 2003 -
Microsoft Document Imaging - see the Office Tools.
Most OCR software makes a dog's breakfast of anything but the most simple
document formatting. The best I have tried is Finereader.
