How do I merge files

I am writing a multi-chapter document, each chapter as a separate file. How
do I merge the files to update footnotes, index and etc.
 
Word is quite able top create huge documents in a single file - which is the
best way to proceed. Use insert > file to create a new document containing
all the chapters.
