How do I fill in option button in word document?

I created a questionaire using MS word. I saved the document and closed it.
However, when I reopened the document, I could not use the drop-down box,
checkbox and all similar fields. I didn't protect the document before, but I
don't know why I can't fill in the form. Please help. Thanks.
 
If this is a form, the form needs to be locked.
