How do I design forms in Outlook 2007?

I designed a number of training related forms in Outlook 2000 and 2003.
After upgrading to Outlook 2007, I can no longer edit the form or related
fields without republishing the entire document. How can I design a form
using a previously modified template?

I'm not sure I understand the problem. In any Outlook version, you'd need to republish after modifying a form.

In the Outlook 2003 version, the design and choose form links were available
under the tools command. I cannot find the link in Office 2007 on any
customized form. In the past I could obtain and modify any published form to
meet my needs. Now I find I must location an original blank document and
republish the form from the beginning. In Office 2007, I cannot edit or
modify many of the fields and the design form option does not change all the
fields even after I edit the form. I tried clearing the cache and the
problem still exists.

Sounds like you need to turn on the developer commands -- Tools | Options | Mail Format | Editor Options | Show Developer tab in Ribbon.
