How do I create my own letter template with drop down menus etc?

I am trying to draft a letter template for a doctor and it needs to have drop
down menu's and areas that the doctor can quickly 'click and type' specific
medications etc.

I am struggling with this, any suggestions?

I am using office 2000 ...

What you are talking about is what Word calls an "online form." Check this
