How do I create a template that will appear in all the tabs below

I am needing a template that will continue to have the form on each of the
sheets, or tabs. I am also needing to do this in multiple incriments

R-click the Spreadsheet Tab, and select "Move or Copy", tick the box for
"Create a copy"
