How do I create a protected police patrol form?

I want to create a Word form which is filled out at the end of a patrol
shift. Users should only be able to enter data in selected fields.

The locked fields would be:
The letter head
Date, Name, Employee #, Log On, Log Off, Unit Plate, Start Km, End Km,
Comments, Alarms, Suspicious Vehicle, etc.

The editable fields would be on the same line, or below, the locked fields.
