How do I collect survey information to a data base with FP03

I would like to collect survey info through my website and save the information to a data base. I want to collect age and a awhole slew of information for people at the onset of their illness.

I've never done this before and was hoping you could tell me how and maybe point mme in the direction of a tutorial or a place where I could find the needed information to make this happen. Thanks.

Depending on what is available on your server you can use ASP, PHP or CGI for this. Alternatively you can buy a solution http://www.perseus.com/survey/software/surveysolutions_standard.html 30 day trial available.
