How can i make an envelope with info for picture packages?

I want to make a template that will allow for printing over the entire
envelope. I want to have picture packages listed with prices and a place to
fill out name, address, phone, playground, etc.
 
This is an MS Access Newsgrop, try reporting you question in the relevant

That does not sound like a task for Access. Try MS Word or MS Publisher

Sorry this is the MS Access (the database program) newsgroup. Try a
newsgroup devoted to the program you are using. You should find people
better able to address your problem there than here.
