How can I get appendix headings to renumber automatically?

I have followed all the directions for numbering appendix headings
automatically, and I can't get it to happen consistently. It keeps starting
over with Appendix A, and I can't figure out what I'm doing wrong.

When you've finished setting up the numbering scheme appropriately,
triple-click on any problematic headings to select the paragraph. Then do
ctrl-q and ctrl-spacebar. That will remove any direct formatting, including
any restart numbering settings.
