How attachments appear

Can anyone tell me why an attachement sometimes appears in the body of the
email and other times it is listed under the subject box?
 
If the file is an image and has been inserted as a picture in an HTML email
message, it will appear in the body of the message **IF** the recipient's mail
reader is capable of reading HTML email. If it is non-HTML or the mail reader
does not support HTML it will appear as an attachment.
