Help designing table based on pre-existing form?

I need some help coming up with a table and the form coding to get something
accomplished. I have a complete database which in all other aspects is
complete. Take a look at this input screen:

I have to keep this screen exactly like it is above, because the users want
it that way for ease of data entry. Don't mind the other tabs on that
screen shot. They are already done and just link to a single table which
has the information like name, date, "call number", cycle, and a bunch of
memo fields for comments (positive and improvement
tabs).

Now, what I have no idea on is how to go about doing the scoring. I know I
need a table of the skills (which I have). With the exception of seven
skills, each skill can only ne 60 or 100. It can't be null or 0. There are
seven skills which could be null.

Please help me. I have no idea where to begin with this scoring...Any
advice would be appreciated. I am pretty "intermediate" with Access, so I
am confident I can accomplish this, I just need some direction.
