Heading numbers are missing in document map

Headings that have page break, i.e. heading appears on top of page, doesn't
display with heading number in document map. Other headings display OK. TOC
looks OK.
 
Remove the manual page break and format the relevant paragraphs as "Page
break before" (Paragraph dialog box) instead. If a certain level of heading
should always start on a new page, you can modify the heading style to
include that formatting. If you need detailed instructions, state your
version of Word.
