If I filter a form and created a report of what I had filtered to my form. Is
it possible to add a note in my report that tells me what was or how the form
was filtered that created such report?

Add an unbound textbox to the report.

Set the control source of that control to something like (untested):

Forms!YourFormName!YourControlName

Note that the form must be open, and the control filled, for the report to
be able to "find" the value.

I expect this is possible. How are you taking a filter criteria from your
form and applying it to the report? Are you using the Filter property of the
report or have you set criteria in the record source query of the report?

I used the record source query, which is also the form query source. I also
found a similar question and a response from Marsh to use expression (date
12/52007):
"=Report.Filter" in a text box. This will work for both me and the user of
my report.
