First, try a web search with terms such as "microsoft access" "guest book" "template"
Second, you don't give any details as to what data you'd like this guest book to have,
but what most people would consider a "normal" guest book should be very simple to build
from scratch. Just one table to hold the info (like a hotel register) with fields for
whatever data you want to capture... and just one form to allow the data entry.
